抑郁症患者家庭功能和心理防御特征研究

The study on function,psychologiocal defense style in patients with depression

目的探讨抑郁症患者应激事件、家庭功能和心理防御方式的临床特征。方法采用生活事件量表、防御方式量表、家庭环境量表中文版,对46例抑郁症患者进行测试,并与40例正常人进行对照。结果(1)抑郁症组生活事件总刺激量[(42.12±32.12)分]高于对照组(P<0.001),其中负性事件刺激量[(33.96±10.26)分]、家庭有关问题[(23.14±12.58)分],工作学习中问题[(16.58±3.20)分]因子分均明高于正常组(P<0.001);(2)抑郁症组家庭环境状况中亲密度[(5.83±2.32)分]、情感表达[(5.04±2.33)分]、矛盾性[(1.37±0.19)分]、独立性[(5.00±2.00)分]、成功性[(5.29±2.07)分]等与常模组对照因子分均明显较低(P<0.05或P<0.001);(3)抑郁症组不成熟型因子分[(4.99±0.91)分],中间型防御因子分[(5.05±2.31)分]明显高于正常组(P<0.001),成熟型因子分[(3.89±1.99)分]明显较正常组低(P<0.01)。结论生活事件尤其是负性事件与抑郁症状发生有密切关系,抑郁症患者的家庭支持系统不良,心理防御方式存在明显的中间型和不成熟倾向。

Objective To explore the clinical characters of stress event, family function and psychological defense style of depressive patients. Methods 46 depressive patients were tested with Life Event Scale , Defense Style Questionnaire, FES-CV . The results were compared with that of 40 normal controls. Results （ 1 ） It was significant that the general scale of life event, the problem concerned with family and the problem about working and study of depressive patients were higher than that of normal controls （ P〈 0. 001 ） . （2） compared to the normal standard , the depressive patients were significant lower in cohesion , expressiveness , conflict , independence , achievement and cultural and active-recreational orientation , organization（ P〈 0.05 or P〈 0. 001 ） . （3） The result showed the greater use of immature defense mechanisms in depressive patients （ P〈 0. 001 ） . the depressive patients scored significant higher on such items as projection , complain , fantasy , withdrawal , somatization and so on （ P 〈 0.05 or P 〈 0. 001 ） , and lower in mature defense （ P 〈 0. 001 ） . Conclusion Life events were concerned with depressive symptom . The depressive patients had poorer family support , and used more immature defense .
