带器妊娠原因分析与相应干预措施

Analysis for the reasons of pregnancy with IUD and corresponding intervention measures

目的找出带器妊娠的主要原因及相应的干预策略。方法分析156例带器妊娠的临床、超声表现,并结合文献加以讨论。结果带器妊娠的原因:(1)宫内节育器(IUD)位置异常占72．4％(113/156),其中IUD下移占66％(103/156),IUD嵌顿占5．8％(9/156),IUD异住占0．6％(1／156)。(2)IUD过期失效占7．0％(1/156)。(3)不能明确原因占20．5％(32/156)。结论IUD位置异常及过期失效是带器妊娠的主要原因,带器妊娠的重点人群是年轻育龄妇女,好发时间是带器一年之内,所以应当采取相应的干预策略。

Objective To explore the main reasons for pregnancy with IUD and corresponding intervention strategies.Methods The clinical and ultrasonic features in 156 cases of pregnancy with IUD were analyzed, as compared with those of literatures. Results The reasons of pregnancy with IUD were as follows： （1） Position abnormality of IUD accounted for 72.4% （113/156）,in which 66% （103/156） were due to IUD downturning,5.8% （9/156） were due to the IUD＇s incarceration and 0.6% （1/156） eases of wrong placement. （2） The IUD expiration accounted for 7.0% （11/156）,（3） Undefined cause accounted for 20.5% （32/156）.Conclusion The main reasons of pregnancy with IUD are position abnormality and IUD expiration.The majority of the people are young reproductive women who are most likely to occure during the first year of using IUD. So we propose the corresponding intervention measures.
