角膜放射状切开术在高度近视眼中的应用

Radial Keratotomy in High Myopia

对73例（144眼）经角膜放射状切开术（RK）治疗并随访一年以上的高度近视眼进行了回顾性分析。术前平均屈光度－7．86D，98％探眼视力≤0．3，术后一年平均降低屈光度6．07D，94％患者裸眼视力改善，52％裸眼视力≥0．5，29％取得正视眼±1．0D以内，过矫＞＋1．0D占2％，低矫＞－1．0D占69％。并发症有术中角膜穿孔8％，眩光感12％，主觉视力波动12％，以及4眼较术前丧失2行最佳矫正视力。

73 consecutive patients (144 eyes) of radial keratotomy(RK) has been analysed retrospectively with thefollow-up period over one year. Before surgery the mean refractive error was -7. 86D spherical equivalent(- 6.0～ - 15. 50 D), and 98% of these eyes had uncorrected visual acuity (VA) of 0. 3 or less. One yearafter surgery, refractive error was reduced by 6. 07 D on the average, 52% of the eyes has an uncorrectedVA of 0. 5 or more, and 29% has achieved within emmetropia±1. 0 D. Undercorrections (> - 1.0D) were69 %, and overcorrections (> +1.0D) with 2%. The major complications were 8% intraoperative corneal perforation, 12% glare one year post surgery, 12% subjective fluctuated change of VA, 4 eyes lost two lines ofcorrected VA. Based on the surgical results, it appeared that the appropriate incision number for RK in highmyopia was 12 and that the redeepening incision was unnecessary.
