运动试验心电图QRS波振幅间期乘积诊断心肌缺血的价值

Value of exercise electrocardiographic detection of myocardial ischemia by simple QRS voltage-duration product

观察经＾９９ｍＴｃ－ＭＩＢＩ心肌灌注断层显像明确有心肌缺血和无心肌缺血病人的运动试验心电图ＱＲＳ波振幅，间期及其乘积的变化。心肌缺血组运动３项指标明显增加；而无心肌缺血组运动后反而一目了然。

Thirty-eight patients who had undergone 99mTc-MIBI myocardial SPECT were studied. Seventeen pa-tients had negative and twenty-one had positive 99mTc-MIBI myocardial SPECT. The QRS voltage, durationand product decreased with exercise in patients withnormal coronary arteries (-1.4 or -2.8mm, -3.4ms and -5.1 or -10.1 mm·ms), but increased inpatients with coronary artery disease(4. 1 or 7. 9 mm,10. 6 ms and 45. 2 or 88. 7mm·ms). Increase in exer-cise-induced QRS voltage-duration product was. betterrelated to number of segmental contraction abnormali-ties than QRS voltage or duration.
