肛管直肠损伤早期的诊断与治疗

Early Diagnosis and Treatment for the Anorectal Injury

目的:改善肛管直肠损伤诊断和治疗的水平。方法:对10年间收治的53例肛管直肠损伤患者的诊断、处理原则、手术方法以及预后进行了总结。53例患者采用结肠造瘘与骶前引流术结合,一期修补,对合并大出血休克的病人在抗休克治疗的同时早期开腹手术,填塞止血或结扎髂内动脉。结果:本组治愈52例。并发盆腔脓肿5例。死于MSOF 1例。结论:早期、正确的诊断和有效的早期处理是治疗肛管直肠损伤的关键。

Objective ： Objective： To investigate the diagnosis and treatment of annrectal injuries （ARI）. Methods. The clinical data of 53 patients with anorectal injuries, such as diagnosis,treatment principal, operation and prognosis, in the recent 10 years were analyzed retrospectively. All of the 53 cases underwent surgical treatment. The colostomy, drainage of sacrum anterior space and primary repair damage tissue were performed. As treatment of shock, the patients with hemorrhage underwent an early abdominal operation. Results： All of 52 patients, were cured, 5 occurred pelvic abscess, but one eventually died of MSOF. Conclusion： early accurate diagnosis and efficient treatment is very important for treatment of ARI.
