芬太尼透皮贴剂治疗癌痛56例观察

The Effect of Fentanyl Transdermal System in Treatment of Cancer Pain: 56 cases Report

目的观察芬太尼透皮贴剂(多瑞吉)治疗中重度癌痛的疗效、毒副作用.方法56例中重度癌痛患者,给予芬太尼透皮贴剂皮肤贴敷,起始剂量2.5 mg,根据疼痛变化每60～72 h调整剂量,直到疼痛基本缓解.观察治疗前后疼痛评分变化、胃肠道反应、肝肾功能、血常规、大小便情况等.结果采用VAS评分法,疼痛完全缓解率53.6%(30/56),有效率83.9%(47/56).胃肠道反应、便秘的发生率分别为3.6%、7.1%.结论芬太尼透皮贴剂可有效治疗中重度癌痛,使用方便,毒副作用轻,选择合适患者能减少毒副反应的发生.

Objective To observe the efficacy and side effects of Fentanyl transdermal system （Durogesic） in the treatment of moderate and severe cancer pain. Methods Fifty six patients with moderate and severe cancer pain were enrolled into this test. Treatment started at a low dose of 2.5 mg, Durogesic was plastered on the skin and replaced every 60- 72 hours with a dose adjusted by the state of pain. The scale of pain, and effects including nausea, changes of function of the grafted liver and kidney, and WBC count were compared before and after the treatment. Result The results were evaluated by VAS scoring method. The pain release rate was 83.9% （47/56）,and the complete response rate was 53.6% （30/56）. The incidences of side effects of nausea and dizziness were 7.1% and 3.6% . Conclusion Fentanyl transdermal system （Durogesic） is effective in relieving cancer pain with lower side effects and convenient in usage. If the choice is right the incidences of side effects can be reduced.
