保乳手术治疗早期乳腺癌

The Treatment of Early-Stage Breast Cancer with Breast-Conserving Operation

目的观察保乳治疗早期乳腺癌的疗效,推广保乳手术加放疗的治疗方法.方法1999年1月-2003年12月,我科共收治23例Ⅰ、Ⅱ期原发性乳腺癌,行保乳手术加术后放射治疗和化疗.结果经随访12～69个月(中位随访期为31个月),除1例于术后27个月出现复发,行改良根治手术,目前无病生存外,其余病例均无局部复发、远处转移和死亡.所有患者于术后1年进行的外观效果评定,优良率为87.0%(20/23).结论早期乳腺癌实施保乳治疗的近期疗效及美容效果满意,可进行广泛临床应用.其远期疗效还需进一步长期观察.

Objective To study the effect of breast-conserving operation for early-stage breast cancer. Methods From January 1999 to December 2003, 23 patients with early-stage （stage Ⅰ or Ⅱ） breast cancer were treated with breast-conserving surgery followed by radical radio therapy and chemotherapy. Results All cases were followed-up. Only 1 case recurred at the 27th month after surgery, who accepted mastectomy surgery followed by chemotherapy and now the survivals without disease. There are no recurrence, distant metastasis or death in the other cases. All the cases were assessed by cosmetic result at 1 year after operation, and the cosmetic results were good in 87.0 % （20/23）. Conclusions The treatment result of breast-conserving surgery followed by radiotherapy is similar to mastectomy, and it is superior to mastectomy in terms of improving breast cosmetics. The long-term result needs more observation.
