海螵蛸对25例尿毒症血液透析患者血磷和甲状旁腺素的影响 被引量:8

Observation on Effects of Cuttlefish Bone on Blood Inorganic Phosphorus and Parathyroid Hormone in 25 Cases of Uremia in Hemodialysis

摘要 目的:观察海螵蛸对尿毒症血液透析患者血磷和甲状旁腺素(PTH)的影响。方法:将74例尿毒症透析治疗的患者随机分成对照组、海螵蛸治疗组和碳酸钙治疗组。对照组不使用任何磷结合剂,海螵蛸治疗组每餐口服海螵蛸3.0g;碳酸钙治疗组每餐嚼服碳酸钙片1.5g,各组观察期3个月。结果:海螵蛸治疗组与对照组和碳酸钙治疗组比较治疗后血磷降低(P<0.01)、PTH 水平降低(P<0.05),血钙升高(P<0.05);海螵蛸治疗组治疗后与治疗前比较血磷降低(P<0.01)、PTH 降低(P<0.05);与碳酸钙治疗组比较其降低血磷效果更优(P<0.05),降低 PTH 水平的疗效相当(P>0.05),对高血钙的影响较少(P<0.05);海螵蛸治疗组与对照组治疗后比较,有明显降低血磷(P<0.01)和 PTH 作用(P<0.01)。结论:中药海螵蛸中碳酸钙含量高,来源丰富,价格便宜,作为磷结合剂替代碳酸钙和醋酸钙,可有效降低血磷,预防和治疗肾性骨病。 Objective:To observe effects of Cuttle-Bone on blood inorganic in hemodialysis. Methods.. 74 cases of uremia in hemodialysis treatment group and a calcium carbonate treatment group. The phosphorus and parathyroid hormone(PTH)in patients of uremia were randomly divided into a control group, a cuttlefish bone control group did not take any conjugation agent of phosphorus. The cuttlefish bone treatment group were treated with oral administration of cuttlefish bone powder 3.0g in each meal and the calcium carbonate treatment group with chewing calcium carbonate tablets 1.5g in each meal. They all were treated for 3 months. Results:After treatment ,blood inorganic phosphorus and PTH decreased(P〈0.01 or P〈0.05)and blood calcium increased(P〈 0. 05)in the cuttlefish bone treatment group as compared with the other two group; blood inorganic phosphorus and PTH decreased as compared with those before treatment (both P〈0.05);the decrease of blood inorganic phosphorus was better than that of the calcium carbonate group (P〈0.05)and the decrease of PHT level was similar to that of the calcium carbonate group (P〉 0. 05), with less effect on blood calcium (P 〈 0. 05). After treatment, blood inorganic phosphorus and PTH decreased significantly in the cuttlefish bone treatment group as compared with those in the control group (both P〈0.01). Conclusion: Cuttlefish bone as a conjugation agent of phosphorus can decrease blood inorganic phosphorus,so as to prevent and treat renal osteopathy.
