摘要 目的探讨同时应用肠内、肠外营养支持对慢性阻塞性肺部疾病Ⅱ型呼吸衰竭患者机械通气的影响。方法将Ⅱ型呼吸衰竭行机械通气患者45例随机分为两组,对照组(n=23例)予以肠外营养,观察组(n=22例)予以肠内、肠外营养,评估通气时间、撤机成功率及血清白蛋白浓度,并观察腹泻、腹胀、呼吸机相关性肺炎的发生率。结果1周后观察组的血清白蛋白浓度及撤机成功率高于对照组(P<0.05),但观察组腹胀发生率比对照组高(P<0.05)。结论同时应用肠内、肠外营养支持能减少Ⅱ型呼吸衰竭患者的机械通气时间,提高撤机的成功率。 Objective To explore the influence of both using intestinal and parenteral nutrition on the mechanical ventilation of patients with type Ⅱ respiratory failure. Method 45 type Ⅱ respiratory failure patients, received invasive mechanical ventilation, were randomly divided into two groups , the control group (n = 23 ) was given parenteral nutrition and the observation group (n = 22) was given both intestinal and parenteral nutrition. After that, the duration of ventilation , rate of successful weaning from ventilation and concentration of plasma albumin were evaluated , meanwhile the incidence of diarrhea, abdominal distention and ventilator associated pneumonia were observed. Result The concentration of plasma albumin and rate of successful weaning were higher in the observation group than in the control group a week later(P 〈 0.05), but the rate of abdominal distention in the observation group was higher than in the control group( P 〈 0.05 ). Conclusion Both using intestinal and parenteral nutrition can short the number of clays of mechanical ventilation for type Ⅱ respiratory failure patients and increase the rate of successful weaning.
