储存式自体输血在老年患者中的应用

Preoperative autologous blood transfusion used in elderly patients

目的探讨老年人术前储存式自体输血的临床效果。方法对我院62例60岁以上的择期手术病例自决定施行储存式自体输血之日起,每天给予口服多糖铁(150mg,每日2次)补充铁剂。在采血前1h按采血量的三倍输注晶、胶体液体(晶、胶体液体量比例为2∶1),经外周静脉输注,然后经另一静脉采血。每次采血400ml,间隔72h采血一次,总采血量不超过1200ml。结果除5例患者因术中出血过多同时输注异体血外,其余患者均未输注异体血,术后恢复良好。结论年龄不是限制老年患者开展储存式自体输血的主要因素,只要患者的血红蛋白(HBG)、红细胞压积(HCT)、血小板(PLT)在正常范围内,无严重心、肺疾患及血液病者均可开展术前储存式自体输血。

Objective To observe the effect of preoperative autologous transfusion in elder patients. Methods The patients took Ferrosi Succinatis 150rag every 12 hours from the day of deciding to use autologous transfusion to the operation day. We drew blood through a CPAD blood bag placed in an antecubital vein. Three times of solution to the blood was simultaneously infused （Crystalloid solution to colloid solution was 2：1）. Results 62 patients received autologous blood transfusions besides 5 patients also received allogenic transfusions because of the heavy blood lost during the operation. All patients had no obvious complication. Conclusions Autologous transfusions can be used with limited indications. It can be used only in patients whose blood HBG; HCT and PLT are normal and without disease of heart and lungs. The procedure is safe and effective in the elder patients.
