关节镜下手术治疗膝盘状软骨损伤

Arthroscopic Management of Discoid Meniscus

目的:探讨盘状软骨损伤的机制和特点,研究其手术时机的选择。方法:对56例行关节镜诊治的盘状软骨病例的临床资料进行分析,探讨盘状软骨患者的病程与疗效之间的关系。对上述资料进行统计学分析。结果:所有盘状软骨损伤患者术后Lysho lm膝关节功能评分都与术前有显著性差异(P<0.01);根据Ikeuch i评分不同病程患者组的术后优良率没有显著差异(P>0.05),而病程小于6个月的患者组与大于6个月的患者组术后膝关节功能恢复为优的比例有显著差异(P<0.05)。结论:对于确诊的盘状软骨患者,应尽可能施行半月板成形术,以达到解除症状并延缓关节退行性变发生的目的。

Objective ：To study the mechanism and feature of discoid meniscus injuries and choice for surgery opportunity. Methods：According to the analysis of 56 cases（60 knees） discoid meniscus diagnozed by arthoscopy, a comparative analysis was made to evaluate the relation of curative effect to course of diseases. Results：There was significant difference in the average Lysholm score of all patients between preoperative and postoperative（P〈0.01 ）. There was not significant difference in tbe patients＇ conditions were graded as excellent and good according to Ikeuchi＇s grading scale between 6 and over 6 mouths of course（P〉0.05）. There was significant difference in the patients＇ conditions were only graded as excellent between 6 and over 6 mouths of course（P〈0.05）. Conclusions ：It is suggested that arthroscopic meniscus plasty plays an important role in the treatment of discoid meniscus of knee, and should be performed possibly as soon as the disease was diagnosed by examination.
