颌面颈部间叶组织恶性肿瘤的超声分析

摘要 目的本文主要研究颌面颈部间叶组织恶性肿瘤,19例的超声诊断要点及误诊原因分析,旨在提高超声诊断符合率.方法使用Logiq-700、DU-5、DU-6超声诊断仪,探头频率8~14 MHz.超声检查主要观察:肿块的形态、质地、内部回声、后方回声、境界和包膜.CDFI观察肿瘤的内部和边缘有血流信号.频谱多普勒测得典型的动脉频谱,静脉频谱和非典型的频谱.结果超声诊断恶性肿块或有恶性可能的肿块13例,符合病理学诊断为68.4%.作者认为诊断的依据是来自灰阶声像图特点,也就是我们看见图像时就观察肿块的各项特点.这样在边检查边思考并结合临床医学知识得出诊断.结论综上所述,超声影像学对于颌面颈部肿瘤的检出率极高,并能作出极有价值的诊断.且与其它影像学诊断相比具有一定的优势,因此作者认为对颌面颈部的肿瘤诊断超声是首选检查方法. Objective By studying the diagnosed features and misdiagnosed causes of 19 cases with facecervical mesenchymal-tissue malignant tumor,our purpose was to increase ultrasound diagnostic correlation with pathology. Methods Instruments of I.ogiq 700, DU-5 and DU-6 (frequency 8 14 MHz)were applied to the observation of tumor's morphogenesis, hardness, echogeneous, posterior echo, margin and capsular. The blood stream signals on the tumor's inside and margin are showed on CDFI ,the typical artery ,typical vein and non-typical artery spectra are measured by Doppler spectrum. Results Malignant tumor or tending malignant rumor were diagnosed by ultrasound in 13 cases, 68.4 percent of corresponding to pathological diagnosis. We think the diagnosis of the tumor comes from observing the every ultrasound character of the tumor cerefully and is also a combination of the synthesized analysis to the ultrasound character and the doctor's exeperience on clinical symptoms and medical knowledge. Conclusions Ultrasound diagnosos has the highest examinal rate in the facecervical tumor,and can make the diagnosis valuable,h has more advantage in comparing with the other imaging diagnosis ,so we consider that ultrasound diagnosis is the first choice in face-cervical tumor.
