摘要
目的:评价LTA、CYFRA211、NSE、CEA4种肿瘤标记物联合检测对肺癌的诊断价值。方法:研究对象分3组,肺癌组、良性肺病(BLD)组、健康对照组。其中肺癌组分为非小细胞肺癌(NSCLC)(鳞癌、腺癌)和小细胞肺癌(SCLC)组。采用乳胶凝集法(LA)检测LTA;放射免疫分析法(IRMA)检测CYFRA211、NSE、CEA。结果:①肺癌组LTA、CYFRA211、NSE和CEA阳性率,显著高于BLD组和健康对照组(P<0.01)。②肺癌组LTA、CYFRA211、NSE和CEA阳性率与肺癌病理类型有关,LTA阳性率以NSCLC组最高(75.6%),CYFRA211以鳞癌组最高(84.6%),CEA以腺癌组最高(81.4%),NSE以SCLC组最高(82.3%),与其他病理类型相比较差异有显著性(P<0.01)。③Ⅲ、Ⅳ期肺癌患者血清LTA、CYFRA211、NSE和CEA阳性率均高于Ⅰ、Ⅱ期(P<0.01)。④LTA、CYFRA211、NSE和CEA4项联检阳性率高于单项与三项检测的阳性率。结论:LTA、CYFRA211、NSE和CEA联检是诊断肺癌有价值的组合方法。
Aim: To evaluate the role of lung tumor-associated antigen( LTA), cytokeratin fragment antigen 21-1 (CYFRA21-1 ) , neuron specific enolase (NSE) , and eareinoembryonie antigen (CEA)in the diagnosis of lung cancer. Methods: LTA, CYFRA21-1, NSE, and CEA were detected in lung cancer (including NSCLS and SCLC) , benign lung disease(BLD) and normal lung tissue. The latex agglutination (LA) assay was performed to detect LTA. Radioimmunoassay was performed to detect CYFRA21-1, NSE,and CEA. Results: The positive rates of LTA, CYFRA21-1, NSE, and CEA in lung cancer tissue were higher than those in BLD and normal lung tissue(P 〈 0.01 ). The positive rates of LTA, CYFRA21-1, NSE, and CEA were correlated with the pathological type of cancer tissue (P 〈 0.01 ) , with the highest positive rate of LTA in NSCLS tissue( 75.6% ) , the highest positive rate of CYFRA21-1 in squamous cell carcinoma( 84.6% ) , the highest positive rate of CEA in adenoeareinoma( 81.4% ) , and the highest positive rate of NSE in SCLC (82.3%) , respectively. The positive rates of LTA, CYFRA21-1, NSE, and CEA at stage Ⅲand Ⅳ were higher than those at stage Ⅰ and Ⅱ ( P 〈 0.01 ). The positive rate of combined detection of LTA,CYFRA21-1 ,NSE,and CEA was higher than the detection of single one or three of the four items together. Conclusion : Combined detection of LTA, CYFRA21-1, NSE, and CEA is a useful method to diagnose lung cancer.
