微创经皮肾穿刺取石术治疗马蹄肾结石

Treatment of calculi within horseshoe kidneys with minimally invasive percutaneous nephrolithotomy

目的:探讨微创经皮肾穿刺取石术治疗马蹄肾结石的可行性与效果.方法:回顾性分析在2001年3月～2004年5月期间接受微创经皮肾穿刺取石术治疗的10例马蹄肾结石患者的疗效及并发症.结果:在10例患者中,4例为双侧马蹄肾结石.本组结石一期清除率为80%(8/10),2例需要二期取石.所有患者均为单通道取石,仅1例患者为上盏通道,其余患者均为中盏通道.平均手术时间110 min,术中平均出血量115 ml,平均住院天数17 d,没有患者需要输血,没有大的并发症出现.结论:采用微创经皮肾穿刺取石术治疗马蹄肾结石是安全有效的.

Objective：To investigate the viability and efficacy of minimally invasive percutaneous nephrolithotomy（MPCNL） in treating calculi within horseshoe kidney. Methods： We retrospectively reviewed the outcome and complications of 10 patients who underwent MPCNL for calculi within horseshoe kidneys from March 2001 to May 2004. Results：Of 10 cases, calculi within bilateral horseshoe kidneys was in 4, 80% （8/10） were rendered stonefree at 1 session. A secon＆look was needed in 2. 1 nephrostomy tract was required in all patients, renal access was obtained through an upper calyx in 1 case, a middle calyx in 9. Average operative time was 110 munites, blood loss volume was 115 ml, average hospitalization stay was 17 days, no blood transfusions were needed in all cases, no major complications were noted. Conclusions：Minimally invasive percutaneous nephrolithotomy is safe and effective in treating calculi within horseshoe kidney.
