口腔颌面部手术后组织缺损的一期修复

摘要 目的探讨应用各种组织瓣和代用品修复口腔颌面部手术后软硬组织缺损的临床应用价值、技术特点及适应症。方法对203例口腔颌面部手术后大、中型组织缺损的患者,立即采用带蒂和游离的各种组织瓣以及自体骨组织和金属代用材料进行舌、口底、面颊部、软腭部、腮腺区、下颌骨等部位的修复重建,并对效果进行观察。结果在203例手术中,成功率为89.7%(182/203),所用各类组织瓣可以较好地修复口腔颌面部的组织缺损。结论立即整复口腔颌面部组织缺损,应根据缺损的组织量、部位、缺损范围和全身情况选用不同的组织瓣和方法。 Objective To study clinical application, the surgical technique characteristics and indications of one - stage plastic and reconstruction of oral and maxillofacial tissue defect by using tissue flaps and metal material, Methods For 203 cases with large and middle oral maxillofacial tissue defects after oral and maxillofacial surgery, the tissue defects regions of tongue, floor of mouth, facial buccal region, soft jaw region, parotid region and mandible were repaired and reconstructed by pedicel flaps and free flaps and bone tissue and metal material .Their repairing effect were observed and analysed. Results Among 203 cases, the successful rate was 89.7 % (182/203) and the repairing effects were satisfied. Conclusion One - stage plastic and reconstruction of tissue defect after oral and maxillofacial surgery was feasibility, but the selection of tissue flaps and methods should depend on the range of defect, site and patient' s general state.
