我院门诊处方审核和干预的初步实施

Initial Implementation of Outpatient Prescription Checking and Intervention in Our Hospital

目的:探讨门诊处方常见问题,评估药师在处方审核和干预中的作用.方法:对我院2004年6月～10月门诊药师日常处方审核工作中所记录的疑义处方进行统计、分类和分析.结果:药师审核和干预的疑义处方平均占所调剂处方的0.35%,处方中用法问题占43.0%,剂量问题占39.4%,总量问题占9.7%,频次问题占2.7%;药师对频次问题的专项干预结果与干预前比较具有显著性差异(P＜0.01);药师干预99%有效.结论:药师在处方审核和合理用药方面起着重要作用.

OBJECTIVE：To investigate the problems commonly occurred in outpatient prescriptions and to evaluate pharmacists＇ role in the checking and intervention of prescriptions.METHODS： Questionable prescriptions recorded by outpatient pharmacists in their routine prescription checking from June, 2004 to October, 2004 were classified and statistically analyzed. RESULTS： On average, the questionable prescriptions accounted for 0.35% of all the prescriptions dispensed by pharmacists. Of the questionable prescriptions,usage problems accounted for 43.0% ,dosage accounted for 39.4%, volume dose accounted for 9.7%, frequency accounted for 2.7% .The results in prescribed frequency were significantly different before and after pharmacists intervention（P 〈 0.01） .The total effective rate of pharmacists＇ intervention was 99.0% . CONCLUSION：Pharmacists play an important role in prescription checking and the rational use of drugs.
