银杏叶提取物对肾病患儿外周血单个核细胞中细胞因子mRNA表达的影响

The Effect of Extract of Ginkgo Biloba Leaf on Inflammatory Cytokines mRNA Expression in Children with Nephrotic Syndrome

摘要 目的探讨银杏叶提取物对原发性肾病综合征(NS)患儿外周血单个核细胞(PBMC)中炎症细胞因子IL-1、IL-6、IL-8和TNF-α的mRNA表达的影响.方法将我科2000年10月~2005年3月收治的35例小儿NS患儿随机分为两组,银杏叶组20例,采用泼尼松加银杏叶提取物治疗;激素组15例,采用常规泼尼松治疗.8周为1个疗程,疗程结束后判断疗效.分别于治疗前、后观察临床和生化指标,同时应用酶联免疫吸附试验(ELISA)检测PBMC培养上清液中IL-1、IL-6、IL-8和TNF-α的含量变化;采用原位逆转录聚合酶链反应(In Situ RT-PCR)检测PBMC中IL-1、IL-6、IL-8和TNF-α的mRNA表达情况.结果治疗8周后,临床症状及血液生化指标明显改善;两组NS患儿PBMC上清液中IL-1、IL-6、IL-8和TNF-α的含量间差别均有显著性意义(P<0.01);两组NS患儿PBMC上清液中IL-1、IL-6、IL-8和TNF-α的mRNA表达量间差别亦均有显著性意义(P<0.01);35例患儿治疗前mRNA均呈强阳性表达,治疗后表达强度降低.结论银杏叶提取物对PBMC中IL-1、IL-6、IL-8和TNF-α蛋白及其mRNA表达有明显的抑制作用,银杏叶提取物治疗NS安全、有效,辅助激素治疗值得临床应用. Objective To evaluate the effects of extracting Ginkgo Biloba leaf on inflammatory cytokines mRNA expression, such as IL -1, IL -6, IL -8 and TNF -α that were secreted by peripheral blood mononuclear cell (PBMC) in children with primary nephritic syndrome ( NS ). Methods 35 children with NS who were hospitalized during October 2000 to March 2005 were treated randomly with two kinds of different treatments. The Ginkgo Biloba leaf group was treated with prednisone plus extract of Ginkgo Biloba leaf, including 20 cases. The prednisone group was treated only with prednisone, including 15 cases. We regarded 8 weeks as a course of treatment, and then observed the therapeutic effects. At the same time, we detected the protein content of IL - 1, IL -6, IL -8 and TNF -α by ELISA in the supernatant fluid of PBMC culture, and detected the mRNA expression of IL - 1, IL - 6, IL - 8 and TNF - α by in Situ RT - PCR. Results After 8 weeks' treatment, the clinical symptom and biochemical assay were ameliorated in Ginkgo Biloba leaf group than that in the prednisone group. The protein content of IL - 1, IL -6, IL - 8 and TNF - α in Ginkgo Biloba leaf group was lower than that in the prednisone group in PBMC ( P 〈0. 01 ). The mRNA expression of IL - 1, IL -6, IL -8 and TNF - α in Ginkgo Biloba leaf group was lower than that in the prednisone group in PBMC. Conclusion The extract of Ginkgo Biloba leaf can inhibit the secretion and mRNA expression of many inflammatory cytokines in PBMC of NS, and has significant therapeutic effects. The extract of Ginkgo Biloba leaf is safe, effective to treat children with NS.
