摘要 目的:应用《数字化注意力测试》对注意缺陷多动障碍儿童进行注意障碍程度评定,采用中国精神障碍分类和诊断标准作为注意力不足的诊断金标准来考核本测试方法是否有一定的诊断价值。方法:于2001-05/2002-04选择了上海第二医科大学附属新华医院上海儿童医学中心儿保门诊的75名儿童,年龄最小的6岁3个月,最大的10岁9个月。用中国精神障碍分类和诊断标准对这些儿童进行评价以明确诊断是否为注意缺陷多动障碍,同时进行数字化注意测试评定。在均值加1个SD之间用M+表示,均值减1个SD之间用M-表示,均值减1SD和2SD之间用M-1.5SD评定,均值减2SD以下用M-2SD表示。结果:75名儿童均进入结果分析,无脱落者。应用《数字化注意力测试》测试,评价患儿中-2SD为10名,中国精神障碍分类和诊断标准单项评价均为注意障碍,符合率为100%。注意力测试在-1.5SD儿童为24名,中国精神障碍分类和诊断标准单项评价注意障碍为17名,占71%。75名就诊学龄儿童应用中国精神障碍分类和诊断标准经过评价后,共有32名儿童(占43%)为注意缺陷多动障碍。而数字化注意测试的结果,符合M-2SD标准者为10名,符合M-1.5SD标准者为24名,共34名,占45%。两种评价经χ2检验比较,差异不显著(P>0.05)。结论:数字化注意力测试适用于对注意缺陷多动障碍儿童的注意障碍测试、评价,特别是在-2SD的情况下,能明确评定有注意障碍。 AIM: To evaluate the severity of inattention in the children with attention deficit hyperactivity disorder by digital attention test, and examine whether the test have certain diagnostic value or not by taking Chinese Classification and diagnostic criteria of Mental Disorders (CCMD) golden standard for inattention. METHODS: Between May 2001 and April 2002, 75 children aged 6 years and 3 months to 10 years and 9 months were selected from the Department of Pediatric Health Care, Shanghai Children's Medical Center, Xinhua Hospital affiliated to Shanghai Second Medical University. CCMD was applied in each child to diagnose if he/she had the ADHD or not, at the same time Digital Attention Test was used. The average value plus less than 1 standard deviation (SD) was expressed as M^+, average value minus less than 1 standard deviation (SD) was expressed as M^-, minus 1SD-2SD as M-1.5SD, minus 2. SD or more as M-2SD. RESULTS: All the 75 children were involved in the analysis of results, no one lost. Ten children whom evaluated as -2SD using digital attention test were 100% matched as Attention limitation by using CCMD single factor criteria. Twenty-four children measured as -1.SSD in digital attention test had about 71% matching in CCMD and seventeen cases had been diagnosed as inattention. Seventy-five school age children evaluated by using both CCDM and digital attention test. Thirty-two children (42.67%) had inattention and ADHD by using CCDM criteria and thirty-four children (45.3%) using digital attention test. There were no significance in chi- square test (P 〉 0.05). CONCLUSION: Digital attention test is suitable for testing and evaluating the inattention in ADHD children, especially the patient during -2SD condition, it can clearly evaluate disturbance of attention.
