单肺移植围术期的监护与处理(附1例报告)

The perioperative management of lung transplantation: a case report

目的:探讨肺移植治疗特发性肺纤维化的围手术期处理要点。方法:对1例双侧特发性肺纤维化伴肺大疱、肺功能重度减损的患者进行左肺移植。结果:术后7d顺利脱机拔管,术后15d胸部CT检查显示左肺扩张良好,左肺血液灌注良好,术后肺功能明显改善,围手术期未发生急性排斥反应,术后49 d出院。结论:充分的术前准备、周密的手术方案、术后严密的监测和及时处理是肺移植手术成功的关键。

Objective. To explore the perioperative management of lung transplantation in idiopathic pulmonary fibrosis. Methods： A 53-year old man with the idiopathic pulmonary fibrosis was accepted a left lung transplantation in our hospital on Apr 11,2005. Results： The patient breaked away ventilator on post-operation day 7, the acute rejection and serious infection were not occurred in the perioperative period. The patient＇s lung function was improved significantly and discharged from hospital 49 days after operation. Conclusions： The key points of operation are full preparation in preoperation, thoughtful plan in operation, strict supervision and management after operation.
