特发性肺纤维化支气管肺泡灌洗液细胞对肺功能的影响 被引量:3

Effect of cells in bronchoalveolar lavage on pulmonary function in idiopathic pulmonary fibrosis

摘要 目的:研究特发性肺纤维化(IPF)患者支气管肺泡灌洗液(BALF)细胞对肺功能的影响。方法:IPF患者32例,安静休息状态下进行肺功能试验(PFTs)和支气管肺泡灌洗(BAL)检查,用HE染色法分析回收的BALF中细胞组分。将BALF中细胞各组分所占百分比与PFTs各项结果进行直线相关分析。结果:肺活量占预计值百分比(%VC)与BALF中嗜酸粒细胞百分比(Eos%)负性相关显著(r=-0.37,P<0.05);单次呼吸法一氧化碳弥散量(DLCO)占预计值百分比(DLCO%)与Eos%成反比(r=-0.47,P<0.01);.V50%、.V25%均与BALF中中性粒细胞百分数(Neu%)负相关明显(r=-0.66,P<0.01及r=-0.45,P<0.01)。结论:IPF肺功能降低与BALF中Neu和Eos有关。 Objective: To investigate the effect of cells in bronchoalveolar lavage fluid (BALF) on the pulmonary function of patients with idiopathic pulmonary fibrosis (IPF). Methods: Thirty-two patients with IPF underwent pulmonary function tests (PFTs) and bronchoalveolar hvage (BAL) at rest. HE staining was performed to analyze the cell composition in BALF. The relationship between the percentage of each cell type and the results of PFTs was analyzed by using linear regression. Results: There was significant negative correlation both between the percentage of eosinophils in BALF and %VC ( r = -0.37, P 〈0.05) and between the percentage of eosinophils in BALF samples and DLCO% ( r = -0.47, P 〈 0.01 ). Both 1/5% ( r = - 0.66, P 〈 0.01 ) and 1/25% ( r = - 0.45, P 〈 0.05) correlated positively with the percentage of neutrophils in BALF. Conclusion: Eosinophils and neutrophils in BALF may contribute to the pulmonary dysfunction in patients with IPF.
