瑞芬太尼用于妇科术后镇痛的临床研究

The clinical study of postoperative analgesia using remifentanil in the gynecology

目的观察瑞芬太尼用于妇科手术后镇痛的安全性和有效性。方法50例择期行附件或子宫切除术患者随机分为实验组和对照组,每组25例。麻醉均采用连续硬膜外麻醉,术中不用任何静脉麻醉药物。实验组(Ⅰ)术后即刻持续静脉输注泵输注低浓度瑞芬太尼,对照组(Ⅱ)术后即刻持续静脉输注泵输注生理盐水,观察术后1、2、4、8、16、24、48、72h的血流动力学、血氧饱和度、呼吸频率、镇痛评分、镇静评分及不良反应等情况。结果实验组与对照组比较,血流动力学平稳,镇痛效果明显,无明显不良反应。结论瑞芬太尼镇痛效果确切,无明显不良反应,可推广应用于临床术后镇痛。

Objective To observe the reliability and effectiveness of postoperative analgesia using remifentanil in the gynecology. Methods 50 cases of ASA Ⅰ- Ⅱ grade hysterectomy or Adnexal ectomy were equally arranged into 2 groups randomly. Two groups all used continuous Epidural anesthesia and there were no any intraoperative intravenous anesthesia drugs be used in the operation . Group Ⅰ continuous infused low concentration remifentanil after operation instantly, and group Ⅱ infused normal saline. The changes of hemodynamics, SPO2 , respiratory rate, visual analogue pain score and Sedative score at 1 hour ,2 hour ,4 hour, 8 hour, 16 hour, 24 hour, 48 hour and 72 hour after operation and the side effects all were recorded. Results Compared with group Ⅱ , In group Ⅰ , the hemedynamics were steady, the effectiveness of analgesia were obvious, and there were no obvious side effects. Conclusions The effectiveness of postoperative analgesia using remifentanil were satisfactory, and there were no obvious side effects, so it could be applied in clinical.
