肝切除、冷冻及射频治疗对肝细胞癌血源性播散影响的研究

Effect of operation,cryotherapy and radiofrequency on hematogenous dissemination of primary hepatocellular carcinoma

目的评价肝切除术、冷冻及射频治疗对肝癌细胞的血源性播散的影响。方法分别于手术前后采取接受肝切除术、冷冻及射频治疗的36例、11例及13例原发性肝癌病人的外周血,提取有核细胞总RNA,采用逆转录后荧光定量聚合酶链式反应(fluorescent quantitative polym erase chainreaction,FQ-PCR)检测外周血甲胎球蛋白(α-fetoprote in,AFP)mRNA。结果肝切除组阳性例数由术前的11例增加到术后的13例,冷冻治疗组由4例增加到5例,射频治疗组无变化;各组的阳性率分别由术前的30.56%、36.36%、38.46%升至术后的36.11%、45.45%、38.46%。经统计学分析AFPmRNA的变化无显著性差异(P>0.05)。结论肝切除术、冷冻及射频治疗没有造成原发性肝癌的血源性扩散,是原发性肝癌可靠的外科治疗方法。

Objective To evaluate effects of hepatectomy, cryotherapy or radiofrequency（RF）on hematogenous dissemination of primary hepatocellular carcinoma（PHC）. Methods mRNA of alpha fetoprotein（AFP） in peripheral blood of 36, 11, 13 patients with PHC treated with operation, cryotherapy and RF respectively was detected by reverse transcription （RT）and fluorescent quantitative polymerase chain reaction（FQ-PCR）pre-and post-operation. Results The patients positive for the AFP mRNA in peripheral blood were increased from 11 to 13 in operative group;from 4 to 5 in cryotherapy group and no changes in RF group. However, there was no significant difference before and after operation（P〉0. 05）. Conclusion Therapies such as operation, cryotherapy or RF on PHC couldn＇t contribute to hematogenous dissemination,which was reliable surgical methods to treat PHC.
