对某医院住院病人抗菌药物围手术期预防性应用的调查

Investigation of Defending Use of Antibacterial Drugs during Perioperation in a Hospital

目的了解某院抗菌药物围手术期预防的现状,并提出管理对策。方法调出2004年12月至2005年5月外科手术科室出院病人的全部病历,按病案号随机抽取315份进行回顾性调查。结果315份病历中,抗菌药物围手术期预防应用率为100%;主要的药物为头孢菌素类、青霉素类、甲硝唑和喹诺酮类抗生素,联合用药占65.7%。仍有在术前和术中不用,而在术后使用,没有把握住用药时机。术后平均用药6天,为时过久。联合用药存在选药不当,标本送检和药敏监测还未受重视。静脉应用率为100%,均为滴注给药,无静脉推注给药。结论抗菌药物围手术预防应用存在问题比较严重,采取有针对性的干预措施刻不容缓。

Objective：To realize the circumstances of antibacterials＇ defending use during perioperation in a hospital,and to give a management measure. Methods：Retrospective study for 315 patients received antibacterials form Dec 2004 to May 2005 was carried out. Results：In the 315 cases,the defending utilization rate of antibacterials was 100%. The antibacterials used most frequently were Cephalosporins,Penicillins,Metronidazole,4-quinolones. The combined use cases were 65.7%. The average period of treatment was 6 days. In some cases,the antibacterials were used in postoperation,not in preoperation or in operation. So the drug use time was incorrect. The drug sorts in combined use weren＇t reasonable, and they still didn＇t pay attention to the sample examining and drug susceptibility inspecting. The vein injection uti- lization rate of antibacterials was 100%. Conclusion ：The problems in defending use of antibacterials during perioperation were serious,and some important intervention measures must be take.
