摘要
目的 探讨如何根据股骨粗隆周围骨折的不同骨折类型选择不同固定,以达最佳疗效.方法应用滑动加压髋螺钉(DHS)9例、角钢板3例、Gamma钉7例、股骨近端髓内钉(PFN)14例、股骨重建钉20例分别治疗此类骨折共53例.结果全部病例均获随访,时间6～27个月,平均12.3个月,骨折全部愈合,髋关节优良49例,占92.5%.结论不同类型的股骨粗隆周围骨折,选择内固定物时应有所倾向.其中,PFN则具有固定牢靠,抗旋转力强,手术操作简单,创伤小,并发症少的优点,是治疗股骨粗隆周围骨折的理想内固定物.
Objective ：To investigate how to choose different internal fixation material according to different femoral peritrochanteric fracture types in order to obtain the curative effect. Methods To treatment the kind of fracture respectively with dynamichip screw （DHS） in 9 cases,angulion plate in 3 cases ,Gamma technique in 7 cases,proximal femoral nail（PFN）in 14 cases and reconstruction femoral interlocking nail in 20 cases,in total 53 cases. Results All patients were followed up for 6-27 months（average 12. 3 months）. The fracture union was obtained for the fractures. The total excellent and good rate was 92.5%. Conclusion The different type femoral peritrochanteric fractures wee would selecte a proper method for it. Among them,PFN has the advantages of rigid fixation,good anti-rotation property,simpler manipulation,and little trauma of opretion. It is an idea interal fixative materinal for peritrochanteric fracuture of femur.
