铁矿井下作业人员动脉顺应性研究

摘要 [目的] 探讨铁矿井下作业人员动脉顺应性的变化,为进一步制定相应的预防与控制措施提供科学依据.[方法] 使用动脉脉搏波形分析仪测量219名南京市某钢铁公司井下作业人员(观察组)的动脉功能:大动脉弹性指数(C1)、小动脉弹性指数(C2)、收缩压(SBP)、舒张压(DBP)、脉压差(PP);用声级计测量井下噪声强度;用听力测定仪测定工人左右耳听力;另取50名南京市不接触职业性有害因素工作人员作为对照组.[结果] 铁矿井下噪声强度为107.2~117.2dB(A),作业人员听力下降率为61.5℅.观察组的C1、C2均明显低于对照组(P<0.05), SBP、DBP则明显高于对照组(P<0.01).听力损失组SBP、DBP、PP均显著高于对照组和听力正常组(P<0.05),而听力损失组的C1、C2则要明显低于对照组.单耳听力下降组、双耳听力下降组的C1、C2均明显低于对照组(P<0.05).分别以C1、C2为因变量,年龄、血压为自变量进行回归分析,发现血压对C1、C2影响很大,其中SBP主要影响C1,DBP主要影响C2.[结论] 铁矿井下作业人员的动脉顺应性降低,血压水平增高,考虑主要与作业环境的强噪声有关. [ Objective] To provide scientific evidence for making proper prevemjve and controlling measures for the arterial compliance of ore miners. [ Methods] The noise of mine was monitored. Aerlition,artcrial compliance including conduction artery elastic index C t, small artery elastic index C2 , systolic pressure., diastolie presstre and pulse rate of 219 miners, in exposed group were measured. 50 workers who were not exposed to the salne working ,conditions were uscd as the control group. [ Results] The noise of working place was 107.2 to 117.2dB (A) 61.5% of workers had impaired hearing. Both C1 and C2 of the exposed group were significantly lower than that of the control group ( P 〈0.05 ), wbile blood pressure ( SBP, DBP) was significantly higher ( P 〈0. 01 ). SBP, DBP and PP (ff the loss of hearing group were also higher than both control group and the normal hearing group (P 〈0. 05 ) , while C1 and C2 were remarkably lower. C1 and C2 of one cat; hearing host group and both ear hearing lost groap were all significantly lower than those uf the cuntrol group ( P 〈 0. 05 ). Taking C1 and C2 as depcndent variable respectively, age and blood pressure as independent variables the regression analysis was carried out. It showed that blood pressure had irffluenec on C1, C2 greatly, SBP mairdy affccted C1, while DBP mainly affected C2. [ Conclusion ] The arterial compliance of the ore miners was reduced, while the blood pressure increased.
