摘要
目的探讨升主动脉开放前不同方法的终末温血灌注在冠脉搭桥手术(CABG)中对心肌的保护作用.方法将40例首次体外循环(CPB)下CABG患者随机分为终末低钾温血灌注组(Ⅰ组)和终末CPB温血灌注组(Ⅱ组),每组各20例.升主动脉开放前3～5min,Ⅰ组用4:1温血低钾停搏液、Ⅱ组用不含停搏液的CPB温血行主动脉根部加血管桥灌注.结果两组升主动脉开放后的心脏复跳率,术后正性肌力药物的应用、机械通气时间和ICU停留时间以及术后24 h肌酸磷酸激酶同工酶水平均无显著差异(P＞0.05).结论温血低钾停搏液和CPB温血终末灌注均有利于CABG患者的心肌保护.
OBJECTIVE To compare the different effect of myocardial protection with two kinds of warm reperfusion methods during coronary artery bypass grafting （CABG）. METHODS 40 patients undergoing CABG were divided into two groups randomly. 20 patients （group Ⅰ） were reperfused with 4：1 warm blood hypokalemia cardioplegia and 20 patients （ group Ⅱ） were reperfused with warm blood before clamp releasing. The effect of myocardial protection of the two methods were evaluated by the percentage of myocardial auto - resuscitation, usage of postoperative inotropic drug, period of postoperative mechanical ventilation and ICU staying, the level of CPK - MB at 24hours postoperation. RESULTS There is no significant different on the effect of myocardial protection between group Ⅰ and Ⅱ. CONCLUSION Both 4： 1 warm blood hypokalemia cardioplegia and warm blood reperfusion provide good effect of myocardial protection in CABG patients.
