应用选择性脾动脉阻断行原位保脾术的探讨

Studies on the operation of spleen reservation in situ by using selective splenic artery blockage(with 8 cases)

应用选择性脾动脉阻断行原位保脾术８例，其中外伤性脾破裂６例，病理脾２例。脾动脉重新开放５例，占６３％，认为脾动脉选择性阻断后，脾内贮血循脾静脉回流人体循环，脾内压降低，脾内含液体成份减少，组织结构纤维成份相对增多，脾脏缩小，富有弹性，缝合时不易发生脾组织切割撕裂，止血彻底，安全可靠。从而解决了保脾手术操作技术上的难题。术后脾动脉重新开放，确保了脾脏的血供。

cases of spleen reservation in situ were operated by using selective splenic artery blockage, Among them 6 cases being trumatic rupture of spleen, 2 cases being pathological damage of spleen and 5 cases being the re-opening of the splenic artery. Selective blockage of the splenic artery is believed to reduce the blood volume in the spleen due to the splenic vein systemic drainage of the stroed blood in the spleen. The reduced pressure and blood volum in the spleen made the sutrue easier and safer with less bleeding, less tissue-cutting and lacerating, which overcame the difficuties in the operation of spleen reservation. The re-opening of the splenic artery assured the blood supply of the spleen.
