下呼吸道不动杆菌属分离株的耐药性分析 被引量:15

摘要 目的了解下呼吸道不动杆菌属的分布及耐药情况,指导临床合理用药。方法取呼吸道感染者的下呼吸道标本进行常规细菌培养鉴定,以K-B法进行药物敏感性试验。结果从3 914份下呼吸道标本中分离出细菌1 649株,其中不动杆菌属227株,占13.8%,总分离率为5.8%(227/3 914),其中鲍曼不动杆菌构成比最高164株,占72.2%(164/227)。分离到不动杆菌最多的3个科室分别为:ICU病房27.8%,呼吸内科26.0%,老干部病房16.7%。在11种抗菌药中,亚胺培南的体外抗菌活性最好,细菌耐药率最低,为8.4%,其次为阿米卡星18.5%、环丙沙星27.8%和头孢哌酮-舒巴坦28.6%。结论亚胺培南是治疗不动杆菌属感染的首选药物。下呼吸道标本中不动杆菌属对复方磺胺甲噁唑、氨曲南、哌拉西林、头孢哌酮的耐药性较严重,不宜选用。 Objective To investigate the resistance of Acinetobacter strains isolated from lower respiratory tract for rational therapy. Methods Acinetobacter strains were isolated from patients with lower respiratory tract infection. Susceptibility testing was performed by K-B methods. Results A total of 1 649 bacterial strains were isolated from 3 914 sputum specimens, of which 227 were Acinetobacter. The isolation rate was 13.8%. The three departments with highest rates were ICU (27.80/00) , Respiratory medicine (26.0% ) and geriatrics (16. 7% ). Imipenem was the most active with the lowest resistance rate (8.4 %) among the 11 antibiotics tested, followed by amikacin ( 18.5 % ) , ciprofloxacin (27. 8 % ) and cefoperazone-sulbactam (28.6 % ). Conclusions Imipenem is the first choice for treating the infection caused by Acinetobacter. Acinetobacter strains isolated from lower respiratory tract are highly resistant to TMP-SMZ, piperacillin, aztreonam and cefoperazone.
