腱球代掌骨头行掌指关节成形的应用研究

Metacarpophalangeal arthroplasty using tendon-bulb to replace metacarpal head

目的:介绍腱球代掌骨头行掌指关节成形术治疗掌指关节损伤的疗效。方法:将受损的掌骨头修整成杯口状,取伤指指浅屈肌腱或掌长肌至腱腹移行处带部分肌膜,制成球状体植入掌骨头缺损处。结果:临床应用24例27指效果满意。术后随访24个月,27指均无疼痛及创伤性关节炎发生。结论:腱球为自体肌腱和腱膜结构,耐缺氧、变形小,植入后能很好地保持掌骨头形状。远期疗效有待于进一步观察。

Objective: To introduce a method using tendon bulb to replace metacarpal head in treatment of metacarpophalangeal joint injuries. Methods: The tendon of superficial digital flexor muscle or palmaris longus tendon was harvested and made into sphere shaped and implanted in the injured metacarpal head that was shaped into a cup. Results: No traumatic arthritis occurred in the patients after operation for 24 months' follow-up. Conclusions: The tendon bulb is an autograft that can bear oxygen deficiency and keep the shape of the metacarpal head.
