近排腕骨切除治疗经舟月骨周围脱位的疗效

Follow-up of proximal row carpectomy to treat trans-scaphoid perilunate dislocation

目的:报道近排腕骨切除术治疗经舟骨月骨周围脱位的临床效果。方法:对1977年6月至1993年6月应用该手术的14名患者,进行功能评估,平均随访6年5个月。结果:11例腕痛消失,3例有轻微疼痛及不适。腕关节掌屈39.9度(,下同),为健侧的58.3%;背屈41.8度,为健侧的65.2%。桡偏8.7度,为健侧的37.7%;尺偏23.1度,为健侧的64.2%。握力达健侧的75.8%。12例恢复了原工作,2例改为轻工作。结论:近排腕骨切除术是治疗经舟骨月骨周围脱位的一种有效、安全的方法。

Objective: To report the effect of proximal row carpectomy for treating trans-scaphoid perilunate dislocation. Methods: From June, 1977 to June, 1993. 14 cases of trans-scaphoid perilunate dislocation were treated by proximal row carpectomy. The follow-up period averaged six years and five months. Results: Wrist pain was completely relieved in 11 cases and 3 wrists had some residual discomfort or minimal pain. The ranges of motion averaged 39.9 degree of flexion, 41.8 degree of extension, 8.7 degree of radial deviation, 23.1 degree of ulnar deviation and accounted for 58.3%, 65.2%, 37.7%, and 64.2% of the opposite side, respectively. The grip strength reached 75.8% of the healthy side. 12 patients returned to their previous work, and 2 changed into lighter job. Conclusions: Proximal row carpectomy is an effective and safe treatment option for trans-scaphoid perilunate dislocation.
