经导管治疗膜周部室间隔缺损的疗效和安全性

Appraisal of the Efficacy and Safety of Transcatheter Occlusion of Perimembranous Ventricular Septal Defects

摘要 [目的]评价经导管介入治疗先天性膜周部室间隔缺损(VSD)的疗效和安全性.[方法]20例VSD患者术前由经胸超声心动图确诊,在透视及经胸超声心动图监视下经导管置入室缺封堵器封堵VSD,术后1、3、6个月分别行经胸超声心动图、心电图评价治疗效果.[结果]20例VSD患者术中经左心室造影明确缺损直径为3~9.5(5.60±2.55)mm,成功封堵20例,封堵成功率100%,手术并发症发生率10%(2/20),封堵术后3个月超声复查左室舒张末径较术前明显缩小.[结论]经导管治疗膜周部VSD是一种成功率较高、并发症少、疗效可靠的介入治疗方法. [Objective]To assess the efficacy and safety of transcatheter closure of perimem branous ventricular septal defects(VSD) . [Methods]Twenty patients were diagnosed by transthoracic echocardiography. To perform the operation, transthoracic echocardiography and X ray were used continuously to monitor the procedure. Transthoracic echocardiography and ECG were performed in 1,3, 6 months after the operation. [Results]The VSD diameter ranged from 3-9.5(5.60-2.55)mm. All VSDs were successfully closed and the successful rate of the operation was 100%, the total complication rate of the operation was 10%. A 3-month follow-up was completed in 17 patients, the median left ventricle end-diastolic dimension had significantly decreased. [Conclusion]Transcatheter closure of perimembranous ventricular septal defects is a good method with a high success rate of placement and less complications and good occlusion effect.
