The expression of LFA-1 and Mac-1 on acute myeloid leukemia cells

摘要 目的探讨淋巴细胞相关抗原-1(LFA-1)(CD11a)和巨噬细胞-1(Mac-1)(CD11b)在急性髓性白血病(AML)细胞淤滞及向组织浸润中的作用及表达,并分析其与临床表现之间的关系。方法采用流式细胞仪检测51例初诊AML患者骨髓白血病细胞膜表面CD11a和CD11b的表达,并与AML患者的FAB亚型、临床表现、实验室指标及治疗缓解率进行相关分析。结果AML细胞上表达的CD11a[(36.476±22.641)%]和CD11b[(26.955±12.328)%]明显高于正常骨髓单个核细胞CD11a[(19.625±12.328)%]和CD11b[(15.513±6.296)%];在AML患者的不同FAB亚型中,CD11a和CD11b的表达没有明显差异,但是在M3中CD11a表达有减低趋势,而在M4和M5中CD11b表达有增高的趋势;相关分析显示,CD11a和CD11b的表达与AML患者的白细胞计数呈明显正相关;阳性细胞≥20%作为CD11a阳性,阳性细胞≥30%作为CD11b阳性,CD11a和CD11b阳性患者的白细胞计数和浸润发生率高于阴性患者,而缓解率低于阴性患者。结论LFA-1和Mac-1在AML患者的白细胞淤滞及向组织浸润中起了一定作用。二者在AML的高表达与患者的白细胞增高和髓外浸润以及缓解率降低有关。因此,LFA-1和Mac-1在AML患者的高表达可能是AML患者预后较差的因素之一。 Objective To evaluate the effects of CD11a and CD11b expression on the hyperleukocytosis and infiltration of AML, and to observe CD11a and CD11b expression on AML blast cells. Methods Flow cytometry was used to investigate CD11a and CD11b expression on AML blast cells; The correlations of CD11a and CD11b expression with the clinical features of AML patients ware evaluated. Results The expression of CD11a and CD11b on AML blast cells from 51 patients were (36.476±22.641)% and (26.955±12.328)%, respectively, and the expression of CD11a and CD11b on normal bone marrow mononuclear cells were(19.625±12.328)% and (15.513±6.296)%, respectively. The expression of CD11a and CD11b were not significantly different on each subtype. The expression of CD11a, CD11b and WBC were significantly positive correlated. In the positive CD11a (≥20 %) and CD11b(≥30 %) expression of the patients, WBC and the rate of infiltration were higher and the rate of CR ware lower than that in the negative CDlla and CDllb expression of the patients. Conclusion The expressions of LFA-1 and Mac-1 on patients of AML were increased significantly, which are related tightly to the some characteristics of AML. The increased expression of LFA- 1 and Mac- 1 on AML are contributed to higher WBC and higher infiltration and lower CR rate. The increased expression of LFA-1 and Mac-1 on AML were considered as a poor prognosis factors.
