观察镜直接推移法建立后腹膜腔行腹腔镜手术的经验

The clinical experience of creating retroperitoneum space by IUPU approach.

目的总结我们用观察镜直接推移法建立后腹膜腔的手术经验。方法对87例患者采用观察镜直接推移法建立后腹膜腔进行后腹腔镜泌尿外科手术的患者进行分析，先用Tmcar直接穿刺法建立通道，后腹膜腔采用观察镜直接分离建立，再用后腹腔镜技术完成所有手术过程。结果87例患者均顺利建立后腹膜腔，平均用时5（3～9．5）min，约出血3～10ml，无明显并发症发生。结论观察镜直接推移法操作简便，耗时少，创伤小，安全可靠，是一种值得推广的方法。

Objective The experience of creating retroperitoneal space by IUPU approacn was reported, Methods There were 87 patients underwent retroperitoneal laparoscopic surgery, all of them were underwent by IUPU approach to enlarge retroperitoneal space. The 10mm trocar were directly implanted in retroperitoneum, the retroperitoneal space were enlarged by laparoscopy, and then retroperitoneum laparoscopic surgery were completed. Resuits The enlarged retroperitoneum space of all 87 patients had been finished successfully. Mean operation time was 5 （3 - 9.5） minites. The estimated blood loss was 3ml - 5ml. No complication occurred. Conclusions The IUPU approach is feasible and safe. Since it is less traumatic and quickly completed, the IUPU approach is worth to popularize.
