宫颈涂片提示炎症963例与阴道镜下宫颈活检结果对比分析

Analysis of the pathological results of biopsy under colposcope in 963 cases of cervical smear showing inflammation

目的通过阴道镜下活检病理结果检验宫颈涂片提示炎症的准确性。方法对963例子宫颈涂片提示炎症的患者行阴道镜下检查,阴道镜下见宫颈上皮病变和异常血管区域者行活检,未见鳞柱状上皮转化区者行宫颈管搔刮,并提供组织病理学诊断。结果组织学检查结果提示宫颈炎症745例,宫颈癌前病变、宫颈癌218例,其中无症状的宫颈癌及癌前病变患者79例(占8.2%)。子宫颈涂片的准确率为77.4%,宫颈癌前病变及宫颈癌假阴性率为22.6%。结论子宫颈涂片提示炎症的病例有必要进一步行阴道镜检查。

Objective To test the accuracy of cervix smear showing inflammation by comparing with the pathological results of biopsy under colposcope. Methods Nine hundred and sixty-three cases of cervix smear showing inflammation were examined with colposcope, the epidermic pathological changes and the abnormal blood vessel area were biopsied, and endocervical curettage was applied on the cases of not seeing cervix epidermic transformation zone. The pathology diagnosis of all the cases was applied. Results Seven hundred and forty-five （77.4%） cases of cervix smear results corresponded to pathological diagnosis. Pseudonegative cervical dysplasia and cervical carcinoma accounted for 22.6%. Cervical dysplasia without symptoms accounted for 8.2%. Conclusion It is suggested that cases of cervical smear showing inflammation should accept colposcope examination.
