摘要
目的:探讨中药复方开心胶囊对大鼠心肌梗死后心室重构的影响。方法:实验于1999-06/12在广州中医药大学临床药理研究所完成。选用SD大鼠70只,并随机分为5组:开心胶囊10g/(kg·d),5g/(kg·d)组、开搏通组、心肌梗死组假手术组。开心胶囊10g/(kg·d),5g/(kg·d)组、开搏通组、心肌梗死组结扎左冠状动脉主干造成心肌梗死后心室重构模型,假手术组除不结扎冠状动脉外,余操作均与其他组动物相同。术后第3天,开心胶囊10g/(kg·d),5g/(kg·d)组分别给予开心胶囊10g/(kg·d),5g/(kg·d)灌胃、开搏通组通过自由饮水饲以开搏通1g/L、心肌梗死组、假手术组饲以正常饮水。连续给药8周采用RM-6000型生理记录仪测定心功能后取心脏,称重及左室重。用全自动图像分析系统分析左心室构型改变。结果:纳入动物70只,由于实验过程中动物死亡,假手术组3只、心肌梗死组5只、开心胶囊10g/(kg·d)组、开心胶囊5g/(kg·d)组、开搏通组各4只在实验过程中脱失,共50只大鼠进入结果分析。①开心胶囊对大鼠左室重量及心脏指数的影响:开心胶囊10g/(kg·d)组、开心胶囊5g/(kg·d)组及开搏通组左室重量及心脏指数均较心肌梗死组减小(P<0.05,P<0.01)。②开心胶囊对左心室构筑的影响:心肌梗死组室壁变薄比低于假手术组(P<0.01),开心胶囊10g/(kg·d),5g/(kg·d)组和开搏通组较心肌梗死组增高(P<0.01),而开心胶囊10g/(kg·d)组及开搏通组作用好于开心胶囊5g/(kg·d)组(P<0.05);心肌梗死组左心腔面积较假手术组增加(P<0.01)。开心胶囊10g/(kg·d),5g/(kg·d)组及开搏通组较心肌梗死组明显减小(P<0.01);心肌梗死组室间隔面积、室间隔厚度增加较假手术组增加(P<0.01),开心胶囊10g/(kg·d)组及开搏通组较心肌梗死组减少(P<0.01),而开心胶囊5g/(kg·d)组未显示统计学意义。③开心胶囊对左心室功能的影响:开心胶囊10g/(kg·d)组与心肌梗死组比较左室内压、左室内压最大上升速率明显升高(P<0.01,0.05),左室舒张末期压明显降低(P<0.05)。结论:心肌梗死可导致心脏构型改变,开心胶囊可以抗心肌梗死后心室重构并改善大鼠心肌梗死后左室收缩和舒张功能。
AIM: To explore the effects of Kaixin capsule on ventricles remodeling of rat after myocardial infarction.
METHODS: The experiment was conducted at the Institute or Clinical Pathology, Guangzhou University of Traditional Chinese Medicine from June to December 1999. Seventy SD rats were selected and randomly assigned into 5 groups: Kaixin capsule 10 g/kg per day and 5 g/kg per day group, captopril group, myocardial infarction group and sham operation group. The rats in the Kaixin capsule 10 g/kg per day and 5 g/kg per day group, captopril group, and myocardial infarction group were ligated left coronary artery main stem to make ventricles remodeling models after myocardial infarction. Those in the sham operation group were treated with the same operation except ligating coronary artery. At the 3^nd day after operation, the rats in the Kaixin capsule 10 g/kg per day and 5 g/kg per day group were given Kaixin capsule 10 g/kg per day and 5 g/kg per day, respectively with gastric perfusion. The rats in the captopril group were fed with 1 g/L captopril and free drinking; Those in the myocardial infarction group and sham operation group were fed with normal drinking. After 8 weeks of continuous administration, cardiac function was recorded with RM-6000 type physiological recorder. After the rats were sacrificed, the hearts were cut out and weigbted;the left ventricular were weighted too. The changes of configuration of left ventricle were analyzed with full automatic image analysis system.
RESULTS: Among the 70 included animals, 3 rats in the sham operation group, 5 rats in the myocardial infarction group, 4 rats in the Kaixin capsule 10 g/kg per day group, 4 rats in the 5 g/kg per day group and 4 rats in the captopril group were dropped out, because of death in the experimental process, thus, totally 50 rats were involved in the result analysis. (1) Effects of Kaixin capsule on weight of left ventricle and cardiac index: The weight of left ventricle and cardiac index in the Kaixin capsule 10 g/kg per day and 5 g/kg per day group and captopril group reduced as compared with the myocardial infarction group (P 〈 0.05,P 〈 0.01 ). (2) Effects of Kaixin capsule on remodeling of left ventricle: Ratio of attenuation of ventricular wall in the myocardial infarction group was lower than that in the sham operation group (P 〈 0.01 ); It increased in the Kaixin capsule 10 g/kg per day and 5 g/kg per day group and captopril group as compared with the myocardial infarction group (P 〈 0.01 ), while the effects of Kaixin capsule 10 g/kg per day group and captopril group were better than that in the Kaixin capsule 5 g/kg per day group (P 〈 0.05). Area of left cardiac chamber in the myocardial infarction group increased as compared with the sham operation group (P 〈 0.01); It decreased significantly in the Kaixin capsule 10 g/kg per day and 5 g/kg per day group and captopril group as compared with the myocardial infarction group (P 〈 0.01 ). Area and depth of interventricular septum in the myocardial infarction group increased as ,compared with the sham operation group (P 〈 0.01 ); It reduced in the Kaixin capsule 10 g/kg per day group and captopril group as compared with the myocardial infarction group (P 〈 0.01 ), while there was no statistical significance in the Kaixin capsule 5 g/kg per day group. (3) Effects of Kaixin capsule on function of left ventricle: Systolic pressure and dp/dt of the left ventricle increased markedly in the Kaixin capsule 10 g/kg per day group and myocardial infarction group (P 〈 0,01.0,05). and the diastasis pressure of left ventricle decreased remarkably (P 〈 0,05 ).
CONCLUSION: Myocardial infarction can lead to the changes of configuration of heart, The Kalxin capsule can anti-remodeling of ventricle after myocardial infarction and ameliorate the functions of contraction and relaxation of left ventricular after myocardial infarction of rats.
