成人肠套叠螺旋CT检查的临床意义

Clinical significance of spiral CT in adult intussusception

目的:探讨螺旋CT检查对成人肠套叠的临床意义。方法:回顾性分析12例成人肠套叠,对其CT表现及病因进行分析。结果:12例均为继发性,共有12处原发病变,其中脂肪瘤2例,淋巴瘤1例,腺瘤性息肉3例,结肠癌2例,憩室1例,血管瘤1例,慢性阑尾炎1例,平滑肌瘤1例。套叠类型:小肠套叠型5例,回回结肠型1例,回结肠型3例,结结肠型3例。12例成人肠套叠,均在套叠部发现肿块状改变,头体部呈分层状靶形肿块改变4例,肾形肿块8例,不规则肿块改变5例。套叠尾部彗星尾征改变8例。结论:成人肠套叠有特征性的CT征象,螺旋CT检查可提高成人肠套叠诊断水平,为临床选择治疗方案提供影像依据,具有重要临床意义。

Objective： To evaluate the clinical significance of spiral CT in adult intussusception. Methods： Spiral CT were performed in 12 cases of adult intussusception. The CT features and location were analyzed, Results： Twelve cases with 12 foci of intussusception were reported. All cases were secondary to primary pathology. Among them, lipoma 2 cases, adenomatous polyp 3 cases, carcinoma of colon 2 cases, diverticulum 1 case, lymphoma 1 case, angioma 1 case, chronic appendicitis 1 case, leiomyoma 1 case. The type of intussusception： small intestine-small intestine 5 cases, ileum-ileum-colon ascendens 1 case, ileum-colon aseendens 3 cases, colon-colon 3 cases. Masses were found in all of the 12 cases, including layering target masses 4 cases, kidney shape masses 8 cases, anomaly masses 5 cases, “Comet-tail” signs 8 cases. Conclusion： Adult intussusception has classic CT features, spiral CT can improve diagnostic ability of intussusception in adult, provide reliable imaging information for selecting treatment plan, has important clinical significance.
