原发性高血压左室舒张功能衰竭患者血B型脑钠肽水平

Left Ventricular Diastolic Dysfunction and Serum Brain Natriuretic Peptide Levels in Patients with Essential Hypertension

目的观察原发性高血压(EH)合并不同程度左室舒张功能衰竭的患者血B型脑钠肽(BNP)水平,探讨BNP水平可否作为高血压左室舒张功能衰竭严重程度的临床判断指标和预后指标。方法筛选高血压合并左室舒张功能衰竭的患者121名,按临床心功能表现分为无心衰组50例、有充血性心衰(CHF)组50例和急性左心衰组21例,用美国博适—Triage干式快速定量心力衰竭/心肌梗死诊断仪进行全血BNP测定。结果高血压左室舒张性衰竭患者,临床有充血性心衰(CHF)组血BNP水平较无心衰组明显增高(P<0.01),而急性左心衰组较另外两组血BNP水平更高(P<0.01)。结论血BNP水平可作为高血压患者左室舒张功能衰竭严重程度的判断指标和预后的判断指标。

Objective To investigate the relation between serum brain natriuretic peptide （BNP） levels and left ventrieular diastolic dysfunction（LVDD） with essential hypertension （EH）. Methods 121patients were categorized into three groups： group 1：50 patients with EH,LVDD, but without clinical symptom; group 2：50 patients with EH,LVDD and congestive heart failure ; group 3：21 patients with EH, LVDD and acute left ventrieular failure. Serum BNP levels were measured in all patients by Triage cardiac panel （ made by Biosite company）. Results The serum BNP levels were higher in group 2 than in group 1 （ P 〈 0.01）, and were higher in group 3 than in other 2 groups （P〈0.01）. Conclusion The levels of serum BNP may be a marker for diagnosis and prognosis of patients with EH and LVDD.
