坚强内固定在下颌骨髁突骨折治疗中的应用

The Clinical Application of Internal Rigid Fixation for the Fracture of Mandibular Condyle Process.

目的:探讨下颌骨髁突骨折坚强内固定术的手术适应证与技术要点。方法:对开放复位坚强内固定手术治疗的92例髁突骨折患者手术前后仔细记录面形、张口受限、面神经损伤及牙合关系紊乱情况,手术前后X线曲面断层片对照骨折复位与愈合牙合状况。结果:髁突骨折X线复查骨折解剖复位率达94.9%,张口受限3例,错牙合1例,钛板断裂1例,面神经暂时性损伤8例。结论:成人髁突骨折明显移位者应用微型钛板内固定可获得满意效果。

Objective： To study the indications and key points of internal rigid fixation techniques for the condylar process fractures. Methods： 92 patients with fractures of condyle process were treated with internal rigid fixtion. The facial outline, restriction of mouth opening, injury of facial nerves and occlusion relationship were carefully recorded before and after the operation. In addition, orthopantomography was used to evaluate the shape, reduction and bone healing of the fracture fragments. Results. Accoliding to X -ray pictures, the rate of anatomical restoration was 94.9% （93/98）. Restriction of mouth opening was seen in 3 cases, mal - occlusion in 1 case, fracture of titanium plates in 1 case,and temporary injury of facial nerves in 8 case. Conclusion ： It is an ideal method to treat fractures of mandibular condyle process with internal rigid fixtion.
