基质金属蛋白酶MMP-1及其抑制因子TIMP-1在子宫内膜异位症中的表达及意义

摘要 目的研究基质金属蛋白酶-1(MMP-1)及其抑制因子TIMP-1mRNA和蛋白在子宫内膜异位症(EM s)异位和在位内膜组织中的表达,探讨其在子宫内膜异位症发生发展中的作用。方法应用反转录聚合酶链反应(RT-PCR)和免疫组化S-P法检测43例EM s的异位和在位内膜及20例正常子宫内膜中MMP-1,TIMP-1mRNA及蛋白的表达水平。结果在EM s异位内膜中MMP-1mRNA及蛋白均呈高表达,TIMP-1mRNA及蛋白均呈低表达,与EM s在位内膜和正常内膜相比,差异有显著性,P<0.05。MMP-1/TIMP-1比值在异位内膜和在位内膜分别为1.46±0.57,1.15±0.38,均显著高于正常内膜(P<0.05);而且异位内膜中比值>1的几率(33/39,84.6%)明显高于在位内膜(P<0.05),两者呈显著负相关。异位内膜组织中MMP-1,TIMP-1mRNA及蛋白的表达无周期性变化,但与r-AFS临床分期有相关性(P<0.05)。结论异位内膜组织中MMP-1高表达,TIMP-1低表达,导致MMP-1/TIMP-1平衡失调,使异位内膜组织具有更强的侵袭能力,在EM s的发生发展中发挥重要作用。 Objective To investigate the expression and the role of matrix metalloproteinase -1 ( MMP -1 ) and tissue inhibitor of matrix metaUoproteinase -1 ( TIMP-1 ) in the pathogenesis of endometriosis. Methods RT-PCR and immunohistochemical S-P were employed to detect the gene and protein of MMP-1 and TIMP-1 in ectopic and eutopic endometrium of 43 patients with endometriosis and 20 normal controls. Results The expression level of gene and protein of MMP-1 in the ectopic endometrium endometrial tissue was higher, while that of TIMP-1 in the endometrial tissue was lower, than that in the eutopic endometria of the patient with endometriosis and the normal controls (P 〈 0. 05). The ratios of MMP-1/TIMP-1 gene in ectopic and eutopic endometria of the endometriosis patients were ( 1.46 + 0. 57 ) and ( 1.15 + 0.38 ) respectively, significantly higher than that of the normal controis ( P 〈 0. 05 ). There was a significantly negative correlation between the expressions of TIMP-1 and MMP -1 in ectopic endometrium (P 〈 0. 05). The expressions of MMP-1 and TIMP-1 in endometrial tissue had no cyclical variation but was relative to clinical stage (P 〈 0. 05). Conclusion The high expression of matrix metaUoproteinase-1 and the low expression of TIMP-1 in endometrial tissue make the ectopic endometrial tissue more invasive, and play an important role in the pathogenesis of endometriosis.
