中药内外治疗对糖尿病足0～1级病变临床疗效的观察

Effects of Chinese Herbal Medicine on the Early Diabetic Foot

目的:观察中药内服、外洗对糖尿病足0～1级病变的影响。方法:共54例,观察肢端症状体征、血液流变学、血流动力学、血浆内皮素、血管多普勒超声等变化,治疗组30例与对照组24例比较。结果:治疗组有效率86.66%,对照组有效率37.50%,有显著差异(P<0.01)。治疗组临床症状体征有显著改善(P<0.01),踝臂血压指数略有提高,血液流变学各指标有不同程度的降低,血浆内皮素明显降低,血糖、血脂、糖化血红蛋白两组治疗后均明显降低。下肢血管超声显示股动脉、动脉内膜有明显改善。结论:中药在糖尿病足0～1级病变的治疗中起一定的干预作用,尤其对临床症状有明显的改善。

Objective To investigate the effect of Chinese herbal medicine on the early diabetic foot. Methods 54 patients of early diabetic foot were divided into two groups, the therapy group （30 cases） and the control group （24 cases）. The clinical symptoms, biochemistry index, hemodynamics, ET, extremity artery color Doppler ultrasound and the speed of nerve conduction were compared in two groups. Results In therapy group chnical symptoms and Anklebrachium index were significantly improved after herbal medication （ P 〈 0.05）, and better than the control group （ P 〈 0.05） .The total effective rate was 86.66% for the therapy group while 37.50% for the control group （P 〈0.01） .The hemorheological parameters in the therapy group were significantly lower than those in the control group （P 〈 0.05 ）. ET, HbAlc, FPG, 2HPG, TG obviously decreased in beth groups （ P 〈 0.05）. The color Doppler ultrasound showed there were obvious difference in intima-media thickness between the two groups （P 〈 0.05 ）. Electromyogram showed there were no significantly changes in the two groups. Conclusion- Chinese herbal medicine given internally and applied externally play a fairly role in the early diabetic foot, especially evidence by the improvement of clinical symptoms.
