低浓度局麻药硬膜外持续输注下舒芬太尼不同模式静脉PCA效应的比较

摘要 目的比较低浓度左旋布比卡因术后硬膜外持续输注下,舒芬太尼不同模式静脉自控镇痛的临床效应.方法妇科择期经腹子宫全切术患者60例(ASA Ⅰ~Ⅲ级),随机分成3组,采用双泵镇痛法,Ⅰ组(n=20):A泵硬膜外持续输注0.2%左旋布比卡因(4 ml/h),B泵为舒芬太尼静脉患者自控镇痛(PCA),采用单一PCA模式,bolus剂量为2μg;Ⅱ组(n=20):A泵同Ⅰ组,B泵用CP模式,即背景剂量1μg/h,bolus剂量为2μg;Ⅲ组(n=20):A泵同Ⅰ组,B泵用CP模式,背景剂量同Ⅱ组,bolus剂量为3 μg;3组锁定时间均为5 min.全程镇痛24 h.结果Ⅱ,Ⅲ组VAS评分及BCS舒适评分优于Ⅰ组,尤其是在术后8 h和12 h差异有显著性(P<0.05,),提示Ⅱ,Ⅲ组镇痛效应明显优于Ⅰ组,而Ⅱ,Ⅲ组间无明显差异;D1/D2比值Ⅰ组(2.7±1.7)高于Ⅱ组(1.2±0.1)和Ⅲ组(1.2±0.1),组间差异明显(P<0.05);另外Ⅰ组有6例(30%)镇痛效果不佳,在术后第6~8小时患者D1/D2比值大于10,需硬膜外追加吗啡1 mg,而Ⅱ,Ⅲ组则各有1例(5%).术后镇痛期间Ramesay镇静评分、下肢肌力恢复情况、肛门排气时间3组差异无显著性(P>0.05).结论左旋布比卡因硬膜外持续输注下,舒芬太尼静脉PCA以CP模式的镇痛效应更好,在此模式中单次剂量建议设置为2μg/bolus.
