摘要 目的评价动态脉压差变化与2型糖尿病早期肾损害的关系。方法105例接受24h动态血压监测的2型糖尿病患者分为糖尿病合并高血压组和正常血压组,每组以动态平均脉压差50mmHg为界,分为正常脉压和高脉压2个亚组,比较不同亚组间微量蛋白尿患者的比例、平均尿白蛋白排泄率、尿微球蛋白等差异。结果在同一组别中,高脉压差亚组微量蛋白尿患者的比例、尿白蛋白排泄率、尿α1鄄MG和β2鄄MG均高于正常脉压差亚组。结论脉压差有助于预测2型糖尿病患者早期肾损害。 Objective To study the influence of dynamic pulse differential pressure on early renal damage in subjects with type 2 diabetes mellitus. Methods One hundred and five patients with type 2 diabetes mellitus received ambulatory blood pressure monitor for 24 hours were divided into 2 groups according to their blood pressure with or without hypertension, each group were further divided into two subgroups with high pulse differential pressure(〉50 mm Hg) and with normal pulse differential pressure ( ≤ 50 mm Hg). Some parameters reflecting early renal damage, including urinary albumin excreting rate, urinary α1-MG and β2-MG levels, were compared among sub-groups. Results When compared between two sub-groups in each group of diabetic patients with or without hypertension, the ratios of patients with microalbuminuria, high urinary albumin excretion rate, high urinary α1-MG and β2-MG were significantly higher in high pulse differential pressure sub-group than in normal pulse pressure sub-group. Conclusion Pulse differential pressure was a good predictive parameter for early renal damage in diabetic patients no matter with or withour hypertension.
