胃癌伴存胆囊病变的外科处理

Surgical management of gallbladder disease in gastric cancer patients

作者报道了３８例胃癌同时伴有胆囊疾病者的胆囊病变的外科处理，３８例占同期６５９例胃癌的５．７７％。３８例胆囊疾病经Ｂ超、手术及病理证实诊断为：慢性胆囊炎胆囊结石症２９例（占７４．３６％）；无结石性慢性胆囊炎６例；胆囊多发性息肉样改变或胆固醇结晶沉积２例；胆囊腺肌瘤１例。３８例中２４例在胃癌手术同时行胆囊手术。作者认为对胃癌伴存的胆囊病变，应根据患者全身情况及胃癌手术治疗的具体情况作出相应的处理，原则上能作胃癌切除的，应同时作胆囊切除术。这种胆囊手术并不增加手术感染的危险性。作者对胃癌伴存胆囊病变的具体手术方案提出５点参考意见。

Abstract In this study 38 out of 657(5.77%) gastric cancer patients were found to have concomitant benign gallbladder disease by laparotomy and pathology, twenty-four patients had surgery of various type directed at their gallbladder disease at the time undergoing gastric cancer operation. It is the authors philosophy to handle the gallbladder disorders Itif the symptoms are evident, and this does not cause significant complications.
