超声心动图对评价经皮冠状动脉内支架植入术后左室舒张功能的变化 被引量:9

Echocardiographic Evaluation of Left Ventricular Diastolic Function After Coronary Stent Implantation of Patients with Coronary Heart Disease

摘要 目的应用超声心动图观察冠心病患者经皮冠状动脉内支架植入术前后左室舒张功能的动态变化。方法冠心病患者及正常对照组各38例,行多普勒组织显像(DTI)及常规超声心动图检查。结果冠心病患者术前各项舒张功能指标均与正常对照组显著不同;术后3d各指标无明显变化;术后3个月及6个月,应用DTI技术检测二尖瓣环舒张早期与晚期主峰速度Em/Am比值较术前显著增大,Teiindex显著下降;而二尖瓣口血流舒张早期与晚期主峰速度E/A比值仅在术后6个月明显改变。结论支架植入术后短期内左室舒张功能未出现明显改善,随着术后时间的延长,左室舒张功能会得到恢复。 Objective To evaluate the effects on left ventricular diastolic function lefore and after coronary stent implantation of patients with coronary heart disease (CAD) via Doppler echocardiography. Methods We examined 38 normals and 38 patients with CAD who were treated with coronary stent implantation. With Doppler tissue imaging (DTI) and pulsed-wave Doppler blood flow, Em, Am, Em/Am, E, A, E/A, IRT, ICT, ET were measured and Tei index (ICT + IRT)/ET were observed. Parameters of left ventricular diastolic function in patients with CAD were obtained before coronary stent implantation, and 3 d, 3 months and 6 months after the operation. Results (1) Compared with the normals, all parameters of the patients with CAD varied significantly before coronary stent implantation. (2) Significant change of Doppler parameters were not found during o and 3 d after coronary stent implantation. (3) Compared with that of before coronary stent implantation, Em/Am and Tel index got significant improvementss during 3 months and 6 months after operation, however E/A increased significantly only in 6 months after operation. Conclusions No significant change of Left ventricular diastolic function was found during the short time after coronary stent implantation. Coronary stent implantation can improve left ventricular diastolic function after a long time of the operation.
