超声诊断胎儿脐带绕颈与临床分析

Ultrasography Diagnosis of Umbilial Cord Around the Neck and Clinical Analysis

目的探讨脐带绕颈的超声诊断价值及临床意义,为临床提供正确的分娩方式。方法对足月妊娠者产前一周内行常规超声检查,将检出的脐带绕颈组与无脐带绕颈组进行产后对比分析。结果超声诊断脐带绕颈的阳性率、阴性率、敏感性、特异性、误诊率、漏诊率及准确率分别为92.75%、97.30%、91.87%、97.61%、7.25%、2.70%、96.17%。脐带绕颈组胎儿宫内窘迫率,胎儿剖宫产率及羊水过少的发生率明显高于无脐带绕颈组(P<0.01)。结论超声诊断脐带绕颈有很高的特异性及准确性,对临床分娩方式的选择,降低围产儿并发症具有指导意义。

Objective To discuss the value of ultrasonographic diagnosis and its clinical practice on umbilical cord around the neck, in order to provide correct analysic way for clinic. Methods The fulll term pregnant wemen have a normal ultrasography one week ahead of prenatal and a contrasted analysis be conducted betwreen the group of umbilical cord around the neck and that of none in clinic. Results The ultrasonography diagnosing rate of umbilical cord around the neck, positive, negative, sensitive, exceptional, misdiagnosis, leaky diagnose and accurate separately is 92.75%, 97.30% , 91.87% , 97.61% , 7.25% , 2.70%, 96. 17%. In the group of umbilical cord around the neck,the rate of fetal intrauterine poverty-stricken,caesarean birth and less of amniotic fluid is obviously higher than that of none （P〈0.01） . Conclusions Ultrasography Diagnosis of umbilical cord around the neck is obviosly exceptional and accurate. It plays a leading role both in reducing complication of infant in perinatal period and choosing clinic chirdbirth way.
