节细胞性副神经节瘤1例报告并文献复习

A clinic pathological analysis for gangliocytic paraganglioma: a case report and review of literature

目的分析节细胞性副神经节瘤的临床病理特点、诊断和鉴别诊断。方法对1例节细胞性副神经节瘤进行光镜、免疫组织化学和电镜观察,并结合文献进行讨论。结果肿瘤组织具有三向分化的特点,由上皮样细胞、梭形细胞和成熟的神经节细胞构成。免疫组化标记上皮样细胞表达CgA、Syn、NSE、NF、P-gp9.5;梭形细胞表达V im、NF、S-100蛋白、P-gp9.5;神经节细胞表达NSE、NF、P-gp9.5。电镜下上皮样细胞胞浆内见神经内分泌颗粒。结论节细胞性副神经节瘤是一种非常罕见的好发于十二指肠的良性肿瘤,临床及病理医师需要提高认识避免误诊,依赖其病理形态学特点结合免疫表型可做出正确诊断。

Objective Objective To analyze the clinic pathological characteristics , the diagnosis and differential diagnosis of gangliocytic paraganglioma （GP）. Methods One case of GP was studied by light microscopy, immunohistochemistry and electronic microscopy. Results The tumor was typically composed of an admixture of epithelial cells, spindle cells and ganglion cells. Immunohistochemically, the epithelial cells were positive for CgA ,Syn, NSE, NF,P- gp9.5 and negative for Vim,Ker,EMA,S -100; the spindle cells were positive for Vim,NF,S- 100,P-gp 9. 5 and negative for Ker, EMA, CgA ,Syn, NSE; the ganglion cells were positive for NSE, NF, P - gp9.5 and negative for Vim,Ker,EMA,CgA,Syn,S - 100. Conclusion GP is a rare, typically benign tumor of the gastrointestinal tract most commonly located in the second portion of the duodenum. The diagnosis is based on its specific morphology and differential diagnosis is very important.
