奥美拉唑、左氧氟沙星、呋喃唑酮三联疗法治疗幽门螺杆菌感染

Triple therapy of omeprezole,levofloxacin and furazolidone combined for Helicobacter pylori eradication

目的:探讨奥美拉唑加左氧氟沙星加呋喃唑酮三联一周疗法根除幽门螺杆菌(Hp)的疗效及安全性。方法:选择100例符合条件的Hp阳性慢性胃炎和消化性溃疡患者,随机分为两组。治疗组采用奥美拉唑(20 mg,2次/天)加左氧氟沙星(200mg,2次/天)加呋喃唑酮(100 mg,2次/天),治疗7天;溃疡患者继用奥美拉唑20 mg,1次/天,3周。对照组采用奥美拉唑(20mg,2次/天)加克拉霉素(250 mg,2次/天)加甲硝唑(400 mg,2次/天),治疗7天;溃疡患者维持治疗同上。疗程结束4周后复查Hp,观察Hp根除率、症状缓解率、溃疡愈合率及不良反应等。结果:治疗组和对照组的症状缓解率、Hp根除率、溃疡治愈率、不良反应发生率分别为93.5%、89.1%、92.9%、21.7%和88.9%、80.0%、93.3%、24.4%。两组差异无显著性(P>0.05)。治疗组与对照组每例根除Hp费用分别为107.82元、130.07元,治疗组根除Hp期望成本比对照组低41.58元。结论:奥美拉唑加左氧氟沙星加呋喃唑酮三联一周疗法是根除Hp的理想方案,可被作为根除Hp一线治疗的选择。

Objective ：To study the efficacy and safety of 1-week triple therapy of omeprazole ilevofloxacin and furazolidone combined for HelicobacterFylori（Hp） eradication. Methods： One hundred patients with Hp infection suffering from chronic gastritis and peptic ulcer were randomized to treatment group and control group. The treatment group were administered omeprazole（ 20 mg, twice a day） , levofloxacin （200 mg, twice a day） and furazolidone（ 100 mg, twice a day） for 1-week, while control group received omeprazole（ 20 mg, twice a day） , clarithromycin（250 mg, twice a day） and metronidazole（ 400 mg, twice a day） for 1-week. The patients with peptic ulcer were treated with omeprazole （20 mg, every day） for another three weeks. Four weeks later, the Hp eradication, symptoms, peptic ulcer healing and side-effects were observed. Results：The Hp eradication rate, symptom ameliorate rate, peptic ulcer healing rate and side- effect rate between the treatment group and control group were 89.1% and 80.0% ,93.5% and 88.9% ,92.5% and 93.5% ,21.74% and 24.44% , respectively. The difference was not significant （ P 〉 0.05 ）. The cost of anti-Hp therapy of each patient was 107.82 Yuan （RMB） and 103.07 Yuan （RMB） in the treatment group and control group respectively. Conclusions： 1-week triple therapy of meprazole,levofloxacin and furazolidone combined is the first choice for Hp eradication.
