中心静脉导管在治疗恶性浆膜腔积液中的临床应用

Clinical application of central venous catheter for treating malignant fluid in serous membrane cavity

目的探讨中心静脉导管在恶性浆膜腔积液的诊断及治疗上的价值。方法通过经皮穿刺置管,抽取积液常规检验及细胞学检查;间断排净积液后分次注入药物。结果胸腔、腹腔及心包共穿刺41针次,全部置管一次成功,通过积液细胞学检查为5例患者明确了诊断,导管平均留置时间17d,1例出现感染。结论中心静脉导管在诊断及治疗恶性浆膜腔积液中使用均安全方便,有良好的推广应用价值。

[Objective] To search the value of central venous catheter in the diagnosis and treatment for malignant fluid in serous membrane cavity. [Methods] The fluid drown from thoracic cavity, abdominal cavity and pericardium by subcutaneous puncture and placing catheter was made the routine examination and cytologic examination after entirely excluding the fluid, the drugs was injected into the cavity. [Results] The punctures were successfully / performed in a total of 41 times in thoracic cavity, abdominal cavity and pericardium. The diagnosis of 5 cases was confirmed by the cytologic examination of the fluid. The catheters were placed for 19 days on average. One patient was infected. [Conclusions] Central venous catheter is placed safely and conveniently in the puncture of thoracic cavity, abdominal cavity and pericardium. The technique is worthy to be widely used.
