“阑尾压痛带征”的临床研究

The clinical study of the zonal pain sign by pressure on the appendix

目的用弹簧称标示重量作“阑尾压痛带征”研究。方法用此“压痛器”对腹部作定位定量施压,每距1cm左右作压痛测试压痛点,将各点连成一线提示阑尾的位置,称为“阑尾压痛带征”。共进行150例临床前瞻性研究。结果150例均能引出“阑尾压痛带征”,其中手术结果与压痛带征全部符合者136例(90.7%),基本符合者9例(6.0%),不符合者5例(3.3%)。经压痛带征检查145例,行阑尾切除手术时间平均43min33s,未进行阑尾压痛带检查的100例,行阑尾切除手术时间平均77min6s。结论压痛带征是存在的,其可提高对阑尾炎的确诊率,缩短阑尾切除手术时间,减少手术创伤,便于寻找阑尾,减少对腹腔脏器的扰乱,节省住院费用,减少不必要的昂贵检查。

Objective To explore the clinical applecation of the zonal pain sign by pressure on the appendix. Methods Using the pressure pain apparratus to make a pressure of orientation and fixed quantity on abdomen, made spots by every 1 cm as a test spot of pressure pain, and then link to every spot that pointed out the location of appendix which was named the zonal pain sign by pain pressure on the appendix , A prospective study with 150 clinical cases was carried on. Results The zonal pain sign by pressure on the appendix can be elicited in 100% of the patients. The results of operation were correspondent with the zonal pain sign entirely on 136 cases （90.7%）, 9 cases were correspondent basicaly （6.0%）, 5 cases were non-respondent（3,3% ）. Vermiform appendix excision was more quickly （43 min 33 s vs 77 min 6 s） ,Conclusion The zonal pain sign by pressure on the apppedix is existent. It may improve the final diagnosis rate of the appendicitis, shorten the time of the operation , decrease the operative impairment and the derangement of the other organs in abdominal cavity , give convenience to find vermiform appendix , spare the cost of hospitalization.
