前交叉韧带重建术后MRI评价和临床相关性研究 被引量:12

MRI Assessment of the Integrity of the Neoligment with Clinical Examination after Anterior Cruciate Ligament Reconstruction

摘要 目的:观察膝关节前交叉韧带重建术后移植物MRI信号强度,与临床检查结果作比较,分析MRI表现与临床的相关性。材料和方法:前交叉韧带重建术后患者44例75次MRI检查,观察移植物形态及信号强度评价移植物的连续性,与临床体检作比较,计算其相关性;并且与第二次关节镜结果比较,计算其相关性。结果:MRI与临床体检的总体相关性为90.7%,6例患者在MRI检查后进行了术后第二次关节镜,两者相关性为100%。结论:MRI评价移植物完整性与临床高度相关,能为ACL重建术后膝关节随访提供重要信息。 To describe the MR appearnace of the neoligament and correlate the MRI with the clinical examination results. Material and Methods: Seventy-five MRI examinations were performed in 44 patients after anterior cruciate ligament reconstruction. Oblique sagittal, coronal and oblique axial images of the neoligament were obtained with serial sequences. The MRI of ACL grafts was categorized into 3 types by signal intensity and continuity of the ligament according to Rak' s method: ①well-defined type, ②intermediate type, ③indiscernible type. The MR appearance of intermediate or indiscernible type was defined as graft tear. Then the results were compared with the clinical examination. Results: For an overall correlation between MRI and clinical examination results of 90.7%. Of the 6 patients who underwent second-look arthroscopy, 100% correlation was present between MRI and arthroscopic results. Conclusion: The high correlation between the MRI and clinical examination proves possibility of MRI assessment of the integrity of the neoligament and provides additional information for the post-operative knee.
